## The Process to Get the In-Home Care You Need

Finding the right care for your loved one comes with a lot of uncertainty and anxiety.

People Care takes the time to listen and educate — walking with you step-by-step through the process, crafting a tailored care plan, and delivering reliable, consistent care.

### Contact People Care

Whether you need short-term help or longer-term care, start by contacting our expert Intake Department. Call 646.964.9827, email \[email protected\], or fill out this form. We’ll get you started the day you do.

### Get Help with Insurance and Home Care Authorization

We’ll help you understand your insurance coverage and assist with the authorization process to receive home care services with People Care. If you’re not already enrolled in Medicaid but are eligible, we can guide you through the process.

Our team is here to answer your financial questions – ensuring you have the information needed to make informed decisions. We’ll partner with the appropriate agencies and departments to help you get the care you need.

### Schedule an In-Home RN Assessment

Once approved for home care services, we’ll schedule an in-person, at-home RN assessment to determine the level of care and scope of services needed.

### Get Your Comprehensive, Tailored In-Home Care Plan

Our RN will work closely with your loved one’s doctor to develop a tailored care plan. We’ll walk through this comprehensive plan with you and your family, answering any questions along the way.

### Meet Your Dedicated Care Team

Based on the personalized care plan, our trained coordination team will select the best-matched home care aide, with consideration for factors like language preferences, desired companionship, and specific care needs. You’ll also have a dedicated coordinator and nurse to ensure seamless care and communication throughout your home care journey.

- **Home Care Aide/Personal Care Aide:** Delivers in-home care according to your tailored care plan
- **Coordinator:** Handles staffing, answers questions and addresses concerns, informs you of your aide and any scheduling adjustments
- **Nurse:** Addresses clinical concerns and oversees the aide to ensure you are receiving the highest level of quality care

### Start Receiving the Care You Need

On the first day of service, you’ll be introduced to your aide, who will follow the written care plan created uniquely for you. This plan will list all of the tasks and activities to be completed at each visit. You’ll receive a folder containing your care plan to keep at your home.

Support with Continuous Improvement

Family members and clients can contact the coordinator or nurse for updates, questions, or to discuss any changes. Contact us at any time to make adjustments based on your needs.
